Notice - Confidential Litigant Information Sheet (CN 10486) Required to Be Filed in All Dissolution (FM) and Non-Dissolution (FD) Matters; Rule Relaxation; Revised CLIS Form
The Supreme Court in its attached May 7, 2024 Order relaxed Rules 5:4-2(g) (Confidential Litigant Information Sheet) and 5:7-4(b) (Cases Enforced by the Probation Division) so as to require the filing of the Confidential Litigant Information Sheet (CLIS) (CN 10486) in all dissolution (FM) and non-dissolution (FD) cases. That requirement is aimed at streamlining case processing, preventing duplicate party records, and improving data accuracy and reporting.
Effective November 8, 2024, eCourts and the Judiciary's electronic document submission system (JEDS) will categorize the CLIS as a required filing with all initial FM or FD complaints and all responses to an initial FM or FD complaint. A deficiency notice will be issued if the CLIS is not filed at the time of the initial filing. If the filing party fails to submit the CLIS within 10 days of receipt of the deficiency notice, their pleading may be dismissed.
Additionally, the CLIS has been revised and reformatted so that parties only provide relevant information for their case. The revised CLIS is attached.

IT IS ORDERED, pursuant to N.J. Const. (1947), Art. VI, sec. 2, par. 3, that, effective immediately and until further order, the provisions of Rule 5:4-2(g) (“Confidential Litigant Information Sheet”) and Rule 5:7-4(b) (“Cases Enforced by the Probation Division”) of the Rules Governing the Courts of the State of New Jersey, are relaxed and supplemented so as to require the filing of the Family Confidential Litigant Information Sheet in every dissolution (FM docket) case and every non-dissolution (FD docket) case filed in the Superior Court. As set forth in those two rules, the Administrative Director of the Courts prescribes the form of the Family Confidential Litigation Information Sheet.
The provisions of this Order shall remain in effect pending adoption of conforming rule amendments.
