Notice – Civil Case Information Statement (Civil CIS) (Rules Appendix XII-B1) – Revisions to Page 4; Addition of a Check Box for Medical Debt Cases; Addition and Removal of Certain Multicounty Litigation Case Types

This notice promulgates revisions to the Civil Case Information Statement ("Civil CIS"), which is included in the Rules of Court as Appendix XII-Bl. The attached version of the Civil CIS reflecting those revisions will be effective January 1, 2024. The revisions, all on Page 4 of the Civil CIS, are as follows:

• The addition of a check box to indicate a medical debt claim.
• The addition of new Multicounty Litigation (MCL) case type 639 involving the Pinnacle Metal-on-Metal (MoM) Hip implants pursuant to the Supreme Court's May 1 7, 2023 Order.
• The removal of six case types for now-concluded MCL designations:
Accutane/Isotretinoin (case type 271); Bristol-Myers Squibb Environmental (case type 281); Stryker Trident Hip Implants (case type 285); Olmesartan Medoxomil Medications/Benicar (case type 299); Firefighter Hearing Loss Litigation (case type 625); and HealthPlus Surgery Center (case type 632).
